UK - Clerical errors are costing nearly three million people up to £250 a year from their state pension, a National Audit Office report shows.
It says the problems have been caused by the failure of employers to complete National Insurance correctly.
The NAO said the majority of people affected were short-term or low paid workers who had been employed in firms with a high turnover.
